74. Height positioning: Align the center of the Hinge slightly above themiddle of the patella (Fig. 3).5. The device is pre-assembled with thick Condyle Pads. There are alsothin and medium Condyle Pads, as well as a hook-and-loop, shimincluded with the device. Use any combination of the Condyle Padsand shim to widen or narrow the width of the device at the knee.6. The supplied tibial wedge pads can be placed between liner to adjustthe fit around the tibia (Fig. 11). They can also be used to position thehinge center forward slightly or removed to position the hinge furtherbackwards (Fig. 12).Fasten the straps in the following order. All straps can be adjustedand/or trimmed to length.7. Fit the Lower Cruciate Strap (B) first, located directly below the knee(Fig. 4). This strap is located above the calf muscle and aids in thecorrect positioning of the device as it helps create the device-to-bonecontact required to help keep the device in place.8. Next, snugly fasten the Bottom Strap (C) by inserting it through theD-Ring.9. Continue with the Upper Cruciate Strap (D), located just above theknee.10. Fasten the Top Strap (E). Make sure that the strap hook tab does notoverlap the elastic insert on the strap.Note: Overtightening the top two straps can cause the device to migrate.The following modifications come pre-installed, if selected during theordering process of the device.Device Application with ACL Cable System1. Apply the device according to the device application procedure. Thefour straps should be fastened, but the secondary cable strap shouldstill be loose.2. Once the device is firmly in place, pass the cable strap through thesmaller D-Ring.3. Tighten the cable strap to place the desired amount of pressure on thetibia.Device Application with PCL Strap Kit1. Remove the Lower Cruciate Strap pad, attach the PCL pad onto thestraps below the knee.2. Apply the device according to the device application procedure.3. Tighten the PCL strap with the leg in full extension so that the strapand pad are comfortably snug. The pad should be centered within theframe. If the PCL strap is too long, trim the strap on the alligator tabside. Replace the alligator tab and fasten. Make sure that the alligatortab is under the pull tab.Device Application with Ski Boot Attachment1. Apply the device according to device application procedure.2. Apply ski boots, ask the patient to bend his knees and lean forward asthough skiing. Position the bottom bearing of the aluminum rod onthe hard plastic shell of the boot and mark the bent leg position of thebearing (Fig. 15).Note: this mark should be positioned to allow for at least ½” (13mm)of plastic shell surrounding it on all sides.3. Remove the boot and device and drill a 3/16th” (5mm) hole at thespot marked on the boot. Use care so as not to drill the tongue andthe shell (Fig. 16).
